The charges stem from a crash on North Main Street in Madisonville on December 1, 2025. Madisonville Police reported that Underwood struck five vehicles, causing significant damage, knocked down a lamppost, and pulled a utility line across the road before his vehicle came to a stop. Officers said they found Underwood unresponsive and bleeding from his mouth, and he was transported to the hospital by EMS. Underwood was taken into custody after he was released from the hospital.

The charges stem from a December 6, 2025, incident during the Christmas parade, when Madisonville Police said Douglas Blair drove through road closures, maneuvered around a patrol car with its emergency lights activated, and entered a crowded intersection at East Arch Street and North Main Street. Officers reported that Blair was under the influence and placed him under arrest.
